Don't forget to bring your insurance information with you.
In order to register and treat you promptly, we request that our patients bring current insurance information with them for each visit, a photo ID and up to date list of all of the medications and past surgical history.
We accept most types of health insurance and file electronically your claim.
Most health plans including HMOs do not require a referral.
